HANDWRITING ASSESSMENT (NDIS)

Service Description:

ASSESSMENT APPOINTMENT: (a) Discussion regarding current observed handwriting issues. (b) Therapist explains how poor handwriting is corrected. (c) Therapist investigates muscle tone and joint range of motion. (d) Client provides a formal handwriting sample (i.e. using the Handwriting Speed Test). (e) Occupational Therapist records clients' posture, shoulder/arm/wrist/hand body mechanics (while writing); pencil pressure; fine-motor fluency. (f) Parent/Guardian emails a copy of the handwriting sample to the Therapist (for "poor formation" and "illegibility" analysis). ASSESSMENT REPORT: A detailed assessment report is emailed to you within 7 days. The report pinpoints areas of concern and provides tailored recommendations to correct the problems. COST BREAKDOWN : NDIS hourly rate: $193.99 (20% DISCOUNT = $155) (a) Treatment Appointment (1 hour = $155). (b) Assessment Report (0.5 hour = $77). TOTAL NDIS COST: $155 + $77 = $232 (NDIS FUNDED). NDIS PLAN MANAGED : Invoice for service is forwarded to your NDIS Plan Manager. NDIS AGENCY MANAGED : Therapist enters services into the NDIS online portal. NDIS SELF-MANAGED: Therapist provides payment details at appointment. Or you can provide credit card details to pay over the phone at the start of the appointment. Therapist provides you with a fully paid invoice within 24 hours of payment (for you to submit to the NDIS for reimbursement).
